Plaque psoriasis close-up on leg. Plaque psoriasis is the most prevalent form of the disease. About 80 percent of those with psoriasis have plaque psoriasis. Its full name is psoriasis vulgaris. The form is characterized by raised, inflamed, red lesions covered by a silvery white scale. It is typically found on elbows, knees, the scalp and the lower back.
